Licensed Clinical Social Worker (LCSW)
Job Description
FSN is seeking an empathetic and driven Clinical Social Worker to join our team. In this role, you will assess the psychosocial needs of patients, develop comprehensive discharge plans, and provide crisis intervention to facilitate a safe and smooth continuum of care.
Key Responsibilities:
- Evaluate patients' social, emotional, and economic needs to determine the best course of care.
- Collaborate closely with doctors, nurses, and physical therapists to coordinate safe and timely patient discharges
- Provide immediate support to patients and families facing grief, trauma, or sudden changes in health status.
- Connect patients with essential community resources, financial assistance, and support groups.
- Act as a liaison between the patient, their family, and the medical team to ensure their care preferences and rights are respected.
- Maintain accurate, timely, and confidential patient records in accordance with hospital policies and regulatory standards.
Qualifications:
- Master of Social Work (MSW) from an accredited university required.
- Current LCSW state licensure
- Minimum of 2 years of social work experience in a hospital setting
- Strong crisis management, excellent communication, and a proven ability to thrive in a fast-paced environment.
Shift & Schedule:
- Day shift: 5x8 or 4x10
