Job Description
Licensed Practical Nurse (LPN) — Long-Term Acute Care Facility
FSN is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team at a Long-Term Acute Care Facility. In this role, you will provide direct patient care under the supervision of Registered Nurses (RNs) and physicians, supporting high-quality care while prioritizing patient safety, comfort, and well-being.
Commitment: 6-month assignment
Schedule: 40 hours/week, 8-hour shifts, 3 weekends per month
Shift hours: 9:45pm to 6:15am
Hourly Pay Rate: Weekday $36, Weekend $37 with a 5% pay increase if extended after 6 months
Orientation & Education Requirement:
You will be expected to attend EVERY day of New Employee Orientation and Nursing Education, Monday–Friday, 8:00am–5:00pm, for approximately 2 weeks.

Key Responsibilities
- Provide direct nursing care, including:
- Monitoring vital signs
- Administering medications
- Assisting with daily patient care needs
- Observe and report changes in patient conditions to the supervising RN or physician
- Assist with patient admissions, discharges, and transfers
- Perform basic wound care, dressing changes, and specimen collection
- Maintain accurate and timely documentation
- Educate patients and families on care plans, medications, and post-discharge instructions
- Collaborate with interdisciplinary teams to support patient outcomes
- Help maintain a clean, safe, and organized patient care environment
Qualifications
- Current, unencumbered state-issued LPN license
- Completion of an accredited practical nursing program
- Basic Life Support (BLS) certification
- Strong clinical and patient care skills
- Excellent communication and interpersonal skills
