Licensed Vocational Nurse (LVN) — State Supported Living Center

FSN is seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to provide essential patient care at a supportive state-operated living center. Position Details:

  • Contract Duration: 6-month assignment
  • Schedule: Full-time, 40 hours/week
  • Shift: 6:00 AM – 2:00 PM
  • Tentative Start Date: 08/01/2026

Key Responsibilities:

  • Deliver direct nursing care, including monitoring vital signs, administering medications, and assisting with daily patient needs.
  • Observe, document, and communicate patient condition changes to supervising RNs or physicians promptly.
  • Support patient admissions, discharges, and transfers, ensuring seamless transitions.
  • Perform basic wound care, dressing changes, and collect specimens accurately.
  • Maintain comprehensive and timely documentation in compliance with facility and legal standards.
  • Educate patients and their families on care plans, medication management, and post-discharge instructions for optimal patient outcomes.
  • Collaborate effectively with interdisciplinary teams to support holistic patient care.
  • Ensure a clean, safe, and organized environment to promote patient safety and comfort.

Qualifications:

  • Current, unencumbered LVN license issued by the state.
  • Graduation from an accredited practical nursing program.
  • At least 2 years of practical nursing experience in a related setting.
  • Valid Basic Life Support (BLS) certification.
  • Strong clinical skills with a focus on patient-centered care.
  • Excellent communication and interpersonal abilities to support team collaboration and patient interaction.
