Licensed Vocational Nurse (LVN) — State Supported Living Center
FSN is seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to provide essential patient care at a supportive state-operated living center. Position Details:
- Contract Duration: 6-month assignment
- Schedule: Full-time, 40 hours/week
- Shift: 6:00 AM – 2:00 PM
- Tentative Start Date: 08/01/2026
Key Responsibilities:
- Deliver direct nursing care, including monitoring vital signs, administering medications, and assisting with daily patient needs.
- Observe, document, and communicate patient condition changes to supervising RNs or physicians promptly.
- Support patient admissions, discharges, and transfers, ensuring seamless transitions.
- Perform basic wound care, dressing changes, and collect specimens accurately.
- Maintain comprehensive and timely documentation in compliance with facility and legal standards.
- Educate patients and their families on care plans, medication management, and post-discharge instructions for optimal patient outcomes.
- Collaborate effectively with interdisciplinary teams to support holistic patient care.
- Ensure a clean, safe, and organized environment to promote patient safety and comfort.
Qualifications:
- Current, unencumbered LVN license issued by the state.
- Graduation from an accredited practical nursing program.
- At least 2 years of practical nursing experience in a related setting.
- Valid Basic Life Support (BLS) certification.
- Strong clinical skills with a focus on patient-centered care.
- Excellent communication and interpersonal abilities to support team collaboration and patient interaction.
