FSN is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to play a vital role in delivering high-quality patient care within our long-term care facility. Key Responsibilities:
- Provide direct patient care, including vital signs monitoring, medication administration, and assisting with daily activities.
- Observe patient conditions closely and report any changes promptly to the supervising RN or physician.
- Support patient admissions, discharges, and transfers with professionalism and empathy.
- Perform routine wound care, dressing changes, and specimen collection with accuracy.
- Maintain accurate documentation of patient information within Electronic Health Record (EHR) systems.
- Educate patients and families on care plans, medications, and post-discharge instructions to ensure understanding and compliance.
- Strictly adhere to infection control protocols, safety procedures, and hospital policies.
- Collaborate with multidisciplinary teams to promote optimal patient outcomes.
- Assist in maintaining a clean, organized, and safe care environment.
Qualifications & Skills:
- Valid and current state-issued Licensed Practical Nurse (LPN) license.
- Graduation from an accredited practical nursing program.
- Basic Life Support (BLS) certification; Advanced Cardiovascular Life Support (ACLS)
- Minimum of 2 years of relevant clinical experience; experience in electrophysiology (EP) lab, cardiology, or cardiac procedures is highly desirable.
- Familiarity with EP procedures and devices such as Pacemakers, AICDs, and cardiac ablation is a plus.
- Proficiency with Electronic Health Records (EHR) systems.
- Strong clinical skills combined with compassionate patient-centered care.
