Licensed Practical Nurse (LPN)

FSN is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide top-quality patient care in a dynamic rehabilitation setting. Join our team and contribute to a patient-centered environment focused on healing and recovery.

Local Candidates: Must reside more than 75 miles from the facility.

Key Responsibilities:

• Deliver safe, effective nursing care to patients on a 41-bed short-term rehab unit specializing in wound care, wound vacs, IV therapy, PICC lines, and nephrostomy care.
• Monitor and assess patient conditions related to orthopedic and cardiac procedures, wounds, hypertension (HTN), congestive heart failure (CHF), and renal failure.
• Administer medications, perform treatments, and assist with special procedures under the supervision of the healthcare team.
• Maintain accurate documentation in EPIC electronic health records.
• Collaborate with interdisciplinary team members to develop and implement individualized care plans.
• Ensure compliance with all safety, health, and institutional policies.

Shift:

• 5 days per week, 2:00 PM - 10:00 PM

Qualifications:

• Valid and current LPN license.
• Minimum of 1 year of nursing experience, preferably in a rehabilitation, wound care, or medical-surgical setting.
• BLS certification required.
• Local candidates not accepted. Candidates must reside more than 75 miles from the facility.
• Experience with wound vacs and IV therapy is preferred.
• Reliable and compassionate with strong communication skills.
