Licensed Practical Nurse – Rehabilitation Unit

Join our team as a Licensed Practical Nurse (LPN) in a dynamic rehabilitation unit that specializes in short-term care for orthopedic and cardiac patients. This facility offers a supportive environment where your skills make a real difference in patients' recovery journeys.

Position Details:

  • Location: 41-bed short-term rehab unit
  • Shift: Multiple shifts available, including weekends on an every-other-week basis
  • Experience Required: Minimum of 1 year of clinical experience
  • Float Coverage: Yes, based on facility needs
  • Patient Ratios:
    • Days: 1:8
    • Nights: 1:20
    • Weekends: 1:10
  • Patient Conditions: Post-operative orthopedic and cardiac procedures, wound care, hypertension, congestive heart failure, renal failure
  • Special Procedures: Wound care, wound vacs, IV therapy, PICC line management, nephrostomy care
  • Equipment: Wound vacs and related devices

Key Responsibilities:

  • Independently manage patient care assignments, supported by RN oversight
  • Administer medications and treatments, including IVs and advanced wound care
  • Utilize the EPIC electronic health record system for documentation and clinical tracking
  • Collaborate effectively with the healthcare team to ensure optimal patient outcomes
  • Maintain compliance with facility protocols and safety standards

Qualifications:

  • Current, active LPN license
  • Proficiency in wound care, IV therapy, and other clinical skills listed
  • Experience with EPIC charting preferred
  • Ability to handle patient assignments with minimal supervision
  • Strong communication skills and a team-oriented approach
