Licensed Practical Nurse – Long-Term Care
Join our dedicated healthcare team as a Licensed Practical Nurse (LPN) in a dynamic long-term care setting. Bring your clinical expertise and compassion to a facility that prioritizes quality patient care and professional growth.
Key Responsibilities:
- Provide hands-on nursing care to residents with diagnoses including dementia, arthritis, CHF, and CVA within a 42-bed long-term care unit.
- Administer medications, wound care, IV therapy, and other specialized procedures with precision and adherence to safety protocols.
- Monitor patient conditions, report changes, and collaborate with the interdisciplinary team to develop and implement care plans.
- Maintain accurate and detailed patient records using EPIC electronic health records system.
- Ensure compliance with all regulatory standards and facility policies to promote a safe and supportive environment.
Qualifications:
- Valid, active LPN license in good standing.
- Candidates must reside more than 75 miles from the facility.
- BLS Certification required.
- Minimum of 1 year of clinical experience in long-term care or related settings.
- Proficiency in wound care, IV therapy, and other relevant clinical skills.
- Experience with EPIC charting required.
- Ability to manage patient assignments independently while working effectively within a team.
- Strong communication skills, professionalism, and a compassionate approach to patient care.
Shift and Work Environment:
- Float staffing available as needed based on facility requirements.
- Weekend coverage is required every other week.
- Staffing ratios: Days - 1:21, Nights - 1:42, Weekends - 1:21.
