Licensed Practical Nurse

We are seeking a compassionate, experienced, and dependable Licensed Practical Nurse (LPN) to join our team. The LPN will deliver high-quality, resident-centered nursing care within a long-term care and rehabilitation setting. This role involves supporting interdisciplinary care plans, collaborating with Registered Nurses, physicians, therapists, and nursing staff to ensure positive patient outcomes. Key Responsibilities:

• Provide direct nursing care to residents and patients, respecting their dignity and promoting comfort.
• Administer prescribed medications and treatments accurately, following facility protocols and physician instructions.
• Monitor and document residents’ vital signs, condition changes, and responses to treatments to ensure timely and effective care.
• Conduct assessments of residents’ physical, mental, and emotional status, reporting any significant changes promptly.
• Assist with admission, transfer, and discharge processes in compliance with regulatory standards.
• Develop meaningful relationships with residents, their families, and the healthcare team to foster a supportive environment.
• Supervise and support Certified Nursing Assistants (CNAs) in delivering consistent, quality resident care.
• Maintain strict adherence to infection control, safety policies, and regulatory requirements.
• Ensure complete and accurate medical records and electronic documentation.
• Participate in care planning meetings, interdisciplinary team discussions, and quality improvement initiatives.
• Respond swiftly and appropriately to resident emergencies and changes in condition to ensure safety and well-being.
• Promote a culture of dignity, respect, and enhanced quality of life for all residents.

Qualifications:

• Current and active Licensed Practical Nurse (LPN) license in the state of employment.
• Minimum of 2 years of relevant nursing experience in a skilled nursing facility, rehabilitation center, long-term care, assisted living, or hospital setting.
• Proficiency in medication administration, nursing procedures, and patient assessments.
• Strong critical thinking and clinical judgment skills.
• Excellent communication and interpersonal abilities.
• Capable of working independently and as part of a multidisciplinary team.
