Overview

As a Registered Nurse Assessment Coordinator, you will lead the Resident Assessment Instrument (RAI) and MDS processes, ensuring precise clinical documentation, regulatory compliance, and optimal resident outcomes. Your expertise will directly impact care quality, reimbursement accuracy, and survey preparedness, making a meaningful difference in the lives of residents.

Key Responsibilities

  • Coordinate and manage the MDS/RAI process in line with federal and state regulations, ensuring timely and accurate assessments.
  • Complete MDS assessments, Care Area Assessments (CAAs), and care plans that accurately reflect resident needs and support regulatory compliance.
  • Ensure documentation supports appropriate coding for reimbursement through thorough review of resident records.
  • Collaborate with interdisciplinary teams to develop, review, and update individualized care plans focused on resident-centered care.
  • Monitor quality metrics related to assessments and care plans, implementing improvement initiatives as needed.
  • Schedule and oversee assessment timelines to guarantee timely submission of MDS data.
  • Participate in Medicare reviews, utilization meetings, and audits, supporting survey readiness.
  • Educate and support nursing staff on documentation standards, regulatory updates, and assessment best practices.
  • Conduct resident assessments and observe changes in condition to inform care adjustments.
  • Analyze clinical outcomes and recommend process enhancements to improve overall care quality.

Qualifications

  • Active RN license in the state of employment.
  • At least 2 years of experience in skilled nursing, long-term care, rehabilitation, or senior care.
  • Minimum of 1 year of MDS/RNAC experience.
  • Strong knowledge of MDS 3.0, RAI processes, and relevant CMS regulations.
  • Excellent organizational, assessment, and documentation skills.
  • Proficiency with Electronic Medical Records (EMR) systems.
  • Critical thinking skills and attention to detail.
