Overview
As an LVN, you will deliver essential nursing services, support seamless patient care transitions, and collaborate with interdisciplinary teams to ensure positive outcomes. Your contributions will directly impact residents’ daily comfort and long-term health.
Responsibilities
- Provide direct nursing care by monitoring vital signs, administering medications, and assisting with activities of daily living
- Observe and report changes in resident conditions promptly to supervising RNs or physicians
- Support patient admissions, discharges, and transfers, ensuring smooth processes
- Perform basic wound care, dressing changes, and collect specimens accurately
- Maintain precise, timely documentation of care activities and interventions
- Educate residents and families on care plans, medications, and post-discharge instructions to foster understanding and compliance
- Collaborate with team members to develop and support individualized care plans
- Follow facility protocols and compliance standards, including ICF-IDD Regulations, DSSLC Policies, and HHSC Standards
Requirements
- Valid, unencumbered state-issued LVN license
- Completion of an approved practical nursing program
- At least 2 years of clinical healthcare experience
- Current Basic Life Support (BLS) certification
- Strong clinical skills focused on safety and quality care
- Excellent communication and interpersonal skills
Shift Availability
- Full-time, 40 hours/week
- Schedule options: 6:00 AM – 2:00 PM or 2:00 PM – 10:00 PM
- Shifts available Sunday through Saturday
