Overview:
We are seeking a dedicated Licensed Practical Nurse to join our behavioral health team in a long-term care setting. Your expertise will support residents' well-being, promote positive behavioral outcomes, and uphold the highest standards of care.

What You'll Do & Achieve:

  • Deliver direct patient care focused on behavioral health needs, utilizing behavior management and de-escalation techniques.
  • Manage clinical tasks such as blood glucose testing, vital sign monitoring, intake and output tracking, and specimen labeling.
  • Implement isolation precautions and maintain safety protocols to ensure a secure environment.
  • Document accurately using EPIC charting to support quality care and compliance.
  • Manage patient assignments independently, coordinating with team members to ensure comprehensive care delivery.
  • Support residents through wound care, IV therapy, and other clinical interventions as needed.

Requirements:

  • Valid, active LPN license in good standing.
  • BLS Certification.
  • At least 2 years of clinical experience in long-term care or related healthcare settings.
  • Proficiency in wound care, IV therapy, and clinical assessments.
  • Experience with EPIC charting preferred.
  • Ability to work independently while effectively collaborating with a team.
  • Strong communication skills, professionalism, and compassionate patient engagement.
