Podiatrist

Hopkinsville, Kentucky ContractLocumPosted 10/26/26Job Number: 498

Job Description

Flagstar Staffing, Inc. is seeking a Kentucky-licensed Podiatrist (DPM) with at least three years of experience to provide on-site podiatry services to the patients and residents of Western State Hospital (WSH) and Western State Nursing Facility (WSNF) in Hopkinsville, Kentucky. WSH is a 175-bed acute care psychiatric facility; WSNF is its 144-bed skilled nursing facility. Both are regulated by The Joint Commission, the Centers for Medicare and Medicaid Services (CMS), and the Kentucky Cabinet for Health and Family Services (CHFS) Office of the Inspector General.
The provider delivers routine and preventive foot care on scheduled monthly clinic days, works under the direction of the facility Medical Director, and is available by phone during weekday business hours for consultation. The population served includes individuals with persistent mental illness, developmental disabilities, and complex or medically fragile conditions, so the role suits a clinician who is comfortable in a psychiatric and long-term care setting and who documents thoroughly.
The position supports a Commonwealth of Kentucky personal service contract administered by the CHFS Department for Behavioral Health, Developmental and Intellectual Disabilities (DBHDID). Kentucky-licensed Advanced Practice Registered Nurses (APRNs) with podiatric foot-care experience will also be considered.

Schedule and commitment

  • WSH: up to two (2) on-site days per month, including at minimum one (1) all-day clinic per month
  • WSNF: up to one (1) on-site day per month
  • Phone consultation: available Monday through Friday, 8:00 AM to 5:00 PM Eastern, by telephone or through an answering service; consultation is by phone only and does not count toward on-site days
  • Scheduling: on-site days are scheduled with the facility based on patient need; there is no guaranteed minimum caseload, workload, or number of hours
  • Term: the contract runs through June 30, 2028, and the Commonwealth may renew it for up to two additional two-year periods

Responsibilities

  • Evaluate and treat podiatric conditions including fungal nail infections, corns, calluses, and ingrown toenails, and perform clipping and debridement of toenails
  • Provide evaluation and routine foot care for patients and residents with diabetes
  • Prescribe medication within the scope of licensure; make treatment recommendations and schedule follow-up visits as needed
  • Refer patients to specialists or other sources when clinically indicated; the facility arranges and pays for any patient transportation
  • Maintain medical records and progress notes on every patient examined; leave an original of all provider notes with the facility for scanning into MediTech and retain a provider copy
  • Consult with facility nursing staff, staff physicians, consultants, and residents’ families on medical care, advisement, and follow-up of injuries or illnesses
  • Participate in training programs for direct care and auxiliary staff when requested by WSH or WSNF
  • Supply the medical equipment and instruments needed to perform podiatry services on site
  • Safeguard each individual’s right to self-determination and participation in treatment; use the least restrictive treatment, with any medical restraint used only under the auspices of the facility human rights committee and in conjunction with WSH/WSNF behavior support specialists
  • Work under the direction of the facility Medical Director and follow DBHDID facility policies and procedures
  • Record on-site hours at each visit to support Flagstar’s monthly service report to the facility, due by the 10th of the following month
  • Maintain HIPAA compliance and meet Kentucky Board of Podiatry, The Joint Commission, CMS, and other applicable licensing requirements and standards throughout the engagement
